Alternative Wellness Services is seeking a motivated and client-centered Care Coordinator to join our southern Maine team. The ideal candidate would have a strong commitment to integrity and value quality care. Keeping in line with a healthy work culture AWS incorporates agency wide activities such as holiday parties, annual all staff days, décor contests and weekly recognition of exceptional staff contributions. Our Care Coordinators work as a part of a team to provide high quality behavioral health services. LMSW-cc and current MSW students are encouraged to apply as we provide required clinical supervision and advancement opportunities. We offer in-house training to meet certification requirements for the position!

Our care coordinators assist clients in improving their lives through a better understanding of how to manage their behavioral health needs, physical health needs and other psychosocial factors that impact their recovery. Duties include, but are not limited to:

  • Completing psychosocial assessments
  • Strengths-based treatment planning
  • Care coordination w/ other providers
  • Health promotion and wellness
  • Client advocacy
  • Client and family support
  • Referral to additional community and social support services
  • Small group facilitation
  • Current MHRT-C or MHRT-C provisional or eligible for a MHRT-C/p paid MHRT-C/p training for the right candidate.
  • Ability to think independently and make decisions
  • Compliance with agency, state and federal guidelines
  • Driving is an essential part of your position and responsibilities
  • Must pass a background check and not be not be on the state or federal suspension and debarment list
  • Must pass criminal and CPS background check
  • Physical Demands: Typing for long periods of time with proper breaks, lifting up to 20 lbs, walking up stairs and standing for periods of time
